1. The advertised plan is not a health insurance policy.
2. The advertised plan provides discounts at certain health
care providers for medical services.
3. The advertised plan does not make payments directly to the
providers of medical services.
4. The advertised plan member is obligated to pay for all health
care services but will receive a discount from those health care
providers who have contracted with the discount plan organization.
5. The corporate name (Affordable Family Health Services, Inc.) and
the locations of the licensed discount medical plan organization.
6. AFHS does not guarantee the
quality of the services or products offered by individual providers.
7. The corporate name (Affordable Family Health Services, Inc.) located at 3955 Pleasantdale Road Ste 102, Atlanta, Georgia 30340 and the locations of this licensed discount medical plan organization are the administrators of this advertised discount plan.
8. Termination of membership: The member may cancel the membership within 30 days after joining the health care card program and the member will receive a refund of all the membership fees paid to the discount health care card program other than money paid as a nominal one-time enrollment fee or money paid by the member to a provider for health care services or products received. All cards must be returned.
